To request reproduction and authorization for use of images, consult Fees for reproduction (Attachment 1) and Reimbursement of costs (Attachment 2), then complete the form below that conforms to the use requested:

A (research and personal use, editable format) .doc Kb 35; B (for publication, exhibition, commercial, professional or other use) .pdf Kb 20;
B (publication, exhibition, commercial, professional, other – downloadable format) .doc Kb 38.
